Primary Graft Dysfunction

So the term for what Gracie's heart is doing is called primary graft dysfunction. This only happens in maybe 25% of heart transplants nationwide and has only happened less than a handful of times in the last 20 years at PCMC. Most of the 25% that had graft dysfunction had other problems like a bad cross match, pulmonary problems etc. Through testing, they found Gracie to have NONE of these problems. Which means...........Gracie is a complete MYSTERY!! We talked to the new heart doctor here, Dr. Kaza (the doctor who went to get Gracie's donor heart). He has personally performed about 60 pediatric heart transplants and has had this happen only ONCE.

Primary Graft Dysfunction
The most frequent cause of death in the first 30 days after transplant is when the new donor heart fails and isn't able to function. This is called primary graft dysfunction. Factors such as shock or trauma to the donor heart or narrowed blood vessels in the recipient's lungs can cause primary graft dysfunction.



They did an echo on her heart yesterday afternoon and found that the left ventricle is still not squeezing. The right ventricle is but the left is not. Really no change from post surgery. They will probably trial her off the ECMO machine on Thursday or Friday. Based on the echo yesterday, our transplant doctor is less optimistic now than she was before. I think all the doctors have hoped that Gracie's left ventricle would improve with time and they have not seen that happen.

Yesterday she was bleeding a lot through the cut down on her wrist. So much so, that they had Dr. K come in and put a few more stitches in it. This morning it had slowed down. We talked to Dr. K for quite a while last night about her situation. He said he was discouraged by the findings of the echo that found her left ventricle was still not working. Anyone who knows Dr. K, knows that 'discouraged' is about as negative as he gets. He seems to be the eternal optimist. He was the only one who thought she had even a slight chance of coming off ECMO the first time around. He also said that the chance of her heart recovering at this point is 'unlikely'.

So, this is what I understand is going to happen. They will trial her off the ECMO machine in the next couple of days. They do not anticipate her heart recovering. If (and it sounds like when) she fails the trial, they will put her back to full function on the ECMO machine and wait for a new heart. In her condition, she would only last on ECMO for maybe two at the most, four weeks while waiting for a new heart. The team is discussing whether or not a VAD (ventricular assist device) would be a good idea for Gracie. This would enable her to wait a lot longer for a heart. Getting a VAD put in is a risky procedure in and of itself so they may not recommend it for Gracie.

Today has been fairly quiet for her. Other than her bleeding, there is nothing else they are working on. It sounds like our only real hope for her is for her to get a new heart. Dr. K said he would take a new heart for her at any time but that she would have a better chance of surviving re-tranplant in a few more days when she is not as swollen etc. So let's pray for round two this weekend!! I'm totally up for another 13 hour surgery ;)

O please please please!!!! This has got to work out. I will pray for a new heart by this weekend. It will happen it's just got to. We will all think very positive for you and your sweetheart. We will keep praying for a new heart for Gracie to come fast. It is obviously the best approach when compared to ECMO or the "Berlin Heart". Don't get me wrong, I am so thankful that Kaidence had her LVAD because it saved her life and she was able to sit up, play and get sronger while on it, but why bring on any more oppertunity for problems, right? We will pray that another family will find comfort and peace at giving Gracie the gift of a new heart.

My heart is aching for you. This is sounding all to familiar. It was Mia's left side that barely moved too. I wish they could trial her off and load her up with milrinone and that her angel heart could hold on. I need to turn the music off to your blog because I can't stop sobbing.

Gracie- you are a miracle! You hold on and wait for a new angel heart! You can do it- and when you do Mia will come visit you and you two can play together- New hearts and all!

For several days, my son Shaun (about Max's age) has been including Gracie in every prayer he says without any prompting from us, even our dinner prayers! He wanted to leave a comment tonight -- "I wish Gracie would get better, but I think that my sister Tera is watching out for her. I know Heavenly Father will take care of her."

That is what I will pray for then. Round 2 this weekend. Dr. K sounds like the nicest man and knowing the staff at Primary's, I don't doubt that he is. Hang in there. I just returned from the Draper temple (minutes ago) and as I'm remembering the celestial room, I feel so comforted for YOU. We are so blessed to have that powerful knowledge of eternal families and the divine plan for us all. Our Heavenly Father is so loving and even though we have doubts and worries, He does not.

I was so nervous about what your next post would be. Although it is definitely serious, I can see the hope shining through. I don't know Dr. K (Dr. Hawkins did our surgery), but he sounds like a great man. I know it probably doesn't mean much, but I have so much hope for Gracie. I see her as a healthy little girl. I just feel like she is going to pull through and be better than ever. You are always in our prayers.

We will add that specific request to our prayers for Gracie. You are also sounding more optimisitc and because of who were dealing with (miracle Gracie) that is understandable!!! We will keep praying along with the hundreds of others. My friend from the ward told me she is praying for her and she told her mom about Gracie and she added her to the prayer roll in the Las Vegas temple and is also adding her to her personal prayers!! We are praying for a new heart!!! I am praying for you and Tom too - I seriously don't know how you are functioning...I feel emotionally drained just as a friend who loves Gracie. It seems she is always the 1% who gets to have the complications...but she is also the miracle girl who came off the ECMO against all odds last time. There is something so incredibly special about Miss Gracie. The music on your blog is so beautiful, but it's sending me over the edge...Good luck with things tomorrow!!! I admit i had not been on since the first part of Jan. My heart ache's for you guys. I will pray for your sweet baby girl..she has always been a miracle and always will be. Amazing that being so tiny, she has been able to touch so many in her trials when many of us live a life without ever making the impact that little Gracie is. What a blessing she is.
